WATER QUALITY FORMS

Below are templates for drinking water warnings, Fecal Coliform/E Coli and Nitrate.  There is also a template for a corrective that resolved the water warning.  

In the table below, there are 13 rows, one for each water system.  Within each row, there are three columns that contain pdf documents, one for Fecal Coliform/E Coli warnings, one for nitrate warnings, and one column for correctives made to the system.  For each type of template, there is one that corresponds for each water system.

To open a document, double-click the pdf icon.  The pdf document will appear on your screen.  You will find 3 to 5 shaded areas on each form.  You will need to fill out these shaded areas before it can be sent out as a public notice.  Holding your mouse over one of the shaded areas will give you a brief description of the type of information that you need to enter in that shaded area.  Click in the shaded area to enter the necessary information.  You can use your tab button to move between the shaded fields.

Once all the shaded fields have been entered, print the form out.  The form then needs to be faxed to the appropriate agencies.  The fax machine in administration has been programmed to send the form to the appropriate agencies.  To fax this form, insert the document, press 01, and press send.  This will automatically send the fax out as a broadcast memo.
